Payments
- AutoPay is our preferred method for payment: keep a credit, debit, or HSA card on file for easy payments.
- Payment for services rendered and copays are required by your insurance carrier to be paid in full at the time of your visit.
- We accept Visa, Mastercard, Discover, cash, and personal checks, or you may use the online bill pay program.
- We offer payment plans under certain circumstances.
- We offer a self-pay discount of 15% for uninsured families. This discount is applicable to office visits only and is valid when the balance is paid in full at the time of service.
Insurance
- Bring a current insurance card for any and all insurances your child has coverage under.
- Claims are filed with any insurance carrier with whom we are participating providers (see list below).
- It is the responsibility of the guarantor to be aware of the eligibility and coverage benefits before an appointment.
- The guarantor is responsible for all charges not paid by the insurance carrier and those that result following a change to the insurance coverage.
- Notify our office as soon as possible of all insurance and address changes.
